ODA Auto-Renew of Membership

Terms & Conditions

The following terms and conditions apply to 12 month payment plans on credit card and ACH as well as auto-renewal one installment payment plans.

a. Membership and Dues. If you have committed to a membership payment plan (e.g., a twelve month plan, or an annual auto-renewal plan), you will automatically be charged each month during that payment plan period or annually (where applicable). You may cancel your membership at any time. As part of the cancellation, your remaining dues amount owing for the current calendar must be paid in full.  (See “Membership Cancellation Policy” below). Stopping recurrent payments does not cancel membership as long as dues are paid in full. Participants in the 12-month payment plan and auto-renew payment plan will be automatically renewed each year until they notify the ODA in writing. The payment allocations you made for the Dental Foundation of Oregon, Dentists of Oregon Political Action Committee, and American Dental Political Action Committee will stay the same in future years until you notify the ODA, in writing, otherwise. A dues statement will be sent each October to notify participants of the amount for the following year’s dues and the first payment or single installment payment will be deducted in January.

b. Payment. Participants in payment plans authorize the ODA to charge their credit card or deduct from their checking account their annual dues, in 12 equal installments, or one installment auto-renewed annually, to take place on the first of each month or first of January where applicable. Payments are applied to the dues year in which they occur (this is not a prepayment plan).  All charges deducted as part of your payment plan are non-refundable. YOU ARE RESPONSIBLE FOR THE TIMELY PAYMENT OF ALL FEES AND FOR PROVIDING ODA WITH A VALID PAYMENT METHOD FOR PAYMENT OF ALL FEES. The ODA currently accepts the following forms of payment: check and credit card (Visa, MasterCard, American Express, and Discover). For your convenience and continuous benefits as a member, if your payment method reaches its expiration date and you do not edit your credit card information or cancel your account, you authorize us to continue billing that credit card on file including extending the expiration date until we are notified by you or the credit card company that the account is no longer valid. We encourage you to constantly update your payment method information or cancel your membership should you wish to discontinue your member benefits. If you have cancelled your membership prior to the end of that payment plan, unless paid in full, your monthly payments will continue until your owing membership dues balance is $0. Further, unless you cancel your membership prior to December 31st of your current payment plan year, at the end of that payment plan, your membership will automatically be renewed for the following year as of January 1st at midnight.

c. Membership Cancellation Policy. IF YOU HAVE COMMITTED TO A MEMBERSHIP PAYMENT PLAN, YOUR MEMBERSHIP WILL CONTINUE UNTIL YOU CANCEL AND, IF YOU CANCEL YOUR MEMBERSHIP BEFORE THE DECEMBER 31ST OF YOUR EXISTING PAYMENT PLAN, YOU WILL BE CHARGED FOR THE REMAINDER OF YOUR OWING DUES AMOUNT BEFORE CANCELLATION CAN BE COMPLETED. To cancel, contact the ODA at members@oregondental.org and cancel the membership by January 1st of the following year at midnight. YOUR MEMBERSHIP WILL CONTINUE UNTIL YOU CANCEL AND, IF YOU DO NOT CANCEL YOUR MEMBERSHIP PRIOR TO THE FIRST DAY OF THE FOLLOWING YEAR AND NEXT MEMBERSHIP PERIOD, YOUR SUBSCRIPTION WILL AUTOMATICALLY RENEW FOR ANOTHER YEAR. All cancellation requests received after January 1st at midnight of the existing payment plan will apply to the following year and next membership period.
